I give PowerFigure Productions ("Producer") and its client Turn Back Time Wellness LLC ("Client") the irrevocable right to photograph and record my likeness, voice and performance on at The Esthetics Studio, 443 Apollo Beach Blvd, Apollo Beach, Florida, and to use, edit, publish and distribute the resulting materials for the Client's marketing and promotional purposes in any media now known or later developed, without further compensation or approval.
I understand this includes the Client's Google and Apple Maps listings, website, brochures and other printed materials, social media, and advertising. I waive any right to inspect or approve the finished materials. I release Producer and Client from claims arising from use consistent with this release, including claims based on editing or alteration made in good faith.
The photographs show me taking part in an assisted stretching session. They will not be used to state or suggest anything about my health, a medical condition, or results I have had.
This release has no expiration unless I write a date in the box below.
